COVID-19 LIABILITY RELEASE FORM

I affirm that, until notified otherwise, the following statements shall apply any time I visit The Manor Salon and Spa.

I affirm that I am to bring and wear a face-mask when attending my appointments. If I fail to bring my own mask, my appointment will be cancelled. 

I affirm that I, as well as my household members, do not currently have, nor have experienced symptoms related to COVID19 (fever, dry cough, fatigue, shortness of breath) within the last 14 days.

I affirm that I, as well as my household members, have not been diagnosed with COVID- 19 within the last 30 days.

I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the last 30 days.

I affirm that I, as well as all household members, have not traveled outside the country, or to any city outside of our own that is or has been considered a "hot spot" for COVID- 19 infections within the last 30 days.

I understand that The Manor Salon and Spa cannot be held liable for any exposure to the virus or any other contagion caused by misinformation on this form or the health history provided by each client.

By signing below I agree to each of the above statements and release The Manor Salon and Spa from any and all liability for the unintentional exposure or harm due to COVID-19.

Providing a Safe Visit

At The Manor Salon and Spa we take your safety very serious. Upon entering our location, you will be required to fill out the above form in advance of each visit. This waiver must be filled out on the day of your appointment. Paper copies are available at the front desk, or you can complete and submit this form digitally.
